Job description Care Coordinator Location: Albrighton, Wolverhampton, WV7 Salary: From 27,000 + bonus & on-call (DOE) Hours: Full-time | Monday to Friday About Us At Apollo Home Healthcare, we provide high-quality, person-centred care to people in the comfort of their own homes. About the Role In this pivotal role, you will be responsible for the planning, coordination, and ongoing management of care packages, ensuring seamless service delivery to our valued clients. You will be based at your local Apollo Home Healthcare office in Albrighton, Wolverhampton, but at times, travel across the West Midlands & Shropshire regions will be required. Key responsibilities include: Function as the first point of contact for our clients, their families, and healthcare professionals. Effectively manage rotas for your care teams, responding efficiently to any day-to-day changes Monitor care delivery to ensure it meets individual care plans and quality standards. Actively participate in multi-agency meetings with Integrated Care Boards (ICBs), advocating for client needs and contributing to care planning decisions. Support the out-of-hours on-call service on a 1 in 8 rota basis - with no requirement for direct care. When covering a weekend, you will receive an additional day off the following week to support a healthy work-life balance. What We're Looking For We are seeking an enthusiastic and outgoing individual with proven care coordination experience. Whether in complex care, domiciliary care, or another setting, you must be able to demonstrate your ability in scheduling, rota coordination, and effective team working. Key requirements include: Full UK Driving Licence and access to your own vehicle. Excellent organisational skills, able to manage multiple care packages and adapt quickly to changing priorities. Confident decision-making, with the ability to think on your feet and solve problems calmly and professionally. Strong problem-solving skills, maintaining composure under pressure. Clear and adaptable communication skills, capable of engaging with families, professionals, and external partners. Skilled in rapport building, ensuring trust and positive relationships with clients and colleagues. Comfortable with negotiation and influencing, particularly when balancing client needs and managing rotas. A proactive team player, with a positive approach and the ability to build strong working relationships.
