We are a friendly, well-established practice with a list size of approximately 13,500 patients, looking to recruit an enthusiastic GP to join our team, with a potential view to future partnership.Currently there are 3 partners supported by a team of 8 salaried GP's, 2 Advanced Care Practitioners, several Nurse Prescribers, Practice Nurses and HCA's, 3 Pharmacists, 2 Pharmacy technicians, a dispensing team, care coordinators and a committed admin and management team.We are a training practice that works with Hull and York Medical School and currently support Year 4 medical students, GP Registrars and FY2's.Main duties of the jobA typical working day includes:Morning clinic: 10 appointments with three protected admin slotsAfternoon clinic: 10 appointments with three protected admin slots15-minute appointments for both face-to-face and telephone consultations-There are no routine home visits during standard clinic sessions. Instead, one GP covers home visits for the practice on a rolling rota basis.-We don't have a Duty GP, instead we build in urgent appointments to all GP's rotas as part of their 20 slots for the day.-Built into the rota each day is a paid, dedicated 30-minute clinical meeting every afternoon, encouraging peer discussion, shared learning, and team support.About us-We pride ourselves on providing a supportive and collaborative working environment with a strong emphasis on continuity of care, with all GPs holding their own patient lists.-We have been rated Good by CQC and are a consistently high QOF achieving practice.-The practice is a very desirable place to work with a relaxed and pleasant atmosphere. It is very important to the partners that this is maintained and developed.Job responsibilitiesProvide high quality patient care including prevention, screening, treatment, management of chronic conditions and health promotionDevelop care and treatment plans in consultation with patients and in line with current Practice protocols as well as wider guidance e.g. NICECarry out surgery and telephone consultations and undertake home visitsSpeak and see patients promptly and in a timely manner. In order to minimise waiting times.Ensure appropriate follow up of patientsGenerate, check and sign acute and repeat prescriptions in-line with practice policyLiaise with other team members and organisations involved in patient care and respond promptly to queriesInvestigate and refer patients as appropriate in line with current guidanceDeal promptly with results and correspondence, clearly communicating any actions that are requiredProvide cover as needed, for all of the above, when other GPs are on leaveWhen providing care ensure the judicious use of resources and NHS fundsKeep clear and contemporaneous computerised consultation notes and patient records to agreed standards and in-line with practice policiesCollect data for audit and QoF purposesPrescribe generically whenever appropriate and in accordance with the Practice formulary, commissioning and national guidanceMaintain high levels of communication with patients and carers to ensure effective delivery of careOther day to day Responsibilities within the Organisation:Demonstrate an awareness of, and compliance with, all relevant Practice policies/guidelines e.g. prescribing, confidentiality, data protection, health and safetyDemonstrate a commitment and contribution to achievement of the highest possible quality standards such as the QOF, prescribing targets and other agreed performance Indicator targetsDemonstrate a commitment and contribution to clinical governance processes including significant event reviewDemonstrate a commitment to life-long learning, audit and evidence-based best practiceContribute to evaluation and audit within the practiceAttend and demonstrate commitment to, clinical, clinical governance and educational meetings, ensure that accurate records and minutes are kept and circulated and that action points from previous meetings have been addressedShow responsiveness to other doctors and team members working with you at any given time with regard to sharing of workloadAssist with the training of other staff members and also with the education of doctors and nurses in training as appropriateDevelop and promote positive collaborative working relationships with partners and all team membersLiaise with all relevant agencies and services to promote knowledge and awareness of services in the practice and local communityComplete patient reports within a timely mannerTake responsibility for own personal development and participate fully in the GP appraisal schemeRaise with management if you identify concerns about the health or performance of a colleague or team memberContribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performanceAlert other team members to issues of quality and riskEffectively manage own time, workload and resources.Review all practice-related e-mails and respond promptly to theseTake responsibility for audits as requiredTake responsibility for targets and services allocated, such as a QOF, Enhanced Services, etc. This includes staying abreast of new guidelines and evidence, developing policies in-line with these and communicating these to the practice team.Provide support to the practice manager as required e.g. complaints or governance issues, CCG/NHSE related issues.Contribute in developing new areas of work, improving existing services and attending team meetings as required.Attend required training events e.g. PLT and feedback key learning points to the teamReporting of Significant Incidents involving the practiceManage own health and stress and alert partners and practice manager promptly if any health issues arise that may impact on personal performanceConfidentialityIn the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. As a GP you will have access to confidential information relating to patients and their carers. You may also have access to information relating to the Practice as a business organisation. All such information from any source is to be regarded as strictly confidential.
