Case Manager

Position Overview: The Case Manager will provide case management services and work with the clinical staff to coordinate care and assist with treatment plans for clients. Responsibilities: Participate in assessments and development of service plans. Implement services according to the individualized service plan (ISP). Collaborate with patients and their families to execute effective service plans, explaining clinical options. Document patient progress and maintain comprehensive clinical records. Coordinate care with the clinical team, providers, and community resources to ensure plan implementation. Assist in maintaining, monitoring, and modifying covered behavioral health services. Identify and seek resources beyond covered services to meet patient needs. Ensure referrals are made and coordinated with service providers; update service plans monthly. Minimum Qualifications: High School Diploma and at least one year of experience in Behavioral Health or Psychiatry. Bachelor’s Degree in a social work-related field preferred. Knowledge of healthcare, detoxification, addiction, co-occurring disorders, DSM criteria, and terminology. Strong reading, writing, speaking, and listening skills; proficient with Microsoft Office. Competencies: Includes job knowledge, communication, problem-solving, decision-making, collaboration, and adaptability to work environment and physical demands. This job operates in a professional office environment with standard equipment and moderate noise levels. Travel is primarily local with some overnight travel expected.
