OverviewStrategic Manager, Integrated Neighbourhood Team - Bosvena Health role hosted by Three Harbours and Bosvena PCN on behalf of the Integrated Neighbourhood Team, working collaboratively with all system partners. There will be an option to work on a secondment basis; applicants should discuss options with their employer. This is a dual-faceted role: initially focused on change management and programme leadership to establish the INT, with a gradual shift towards overseeing day-to-day operational delivery and continuous improvement. The INT aims to deliver integrated, person-centred care, reduce health inequalities, and improve population health outcomes. The initial primary piece of work will be a change management programme to align key service providers within the INT to become a cohesive and high-functioning frailty team. You will bring together workforce from primary and community teams and work to support the Clinical Leadership from the Frailty Lead GP, linking with hospital Eldercare Consultants to create hospital in-reach and pull services to ensure that people return to their own bed as soon as possible. This role involves wide-MDT collaboration with local organisations who may support people within the community. Key ResponsibilitiesStrategic Leadership & Change Management Adopt and apply the principle of the NHS 10-year plan to working within the community at place and lead on integration and shared goals. Demonstrate knowledge of the INT population, using population health data to extract reports, data and analytics for wider system partners and commissioners. Lead the co-design and development of the INT model, strengthening a leadership team with system partners and local communities. Act as the key point of coordination for all INT development activities, facilitating collaboration across organisations and ensuring alignment with ICS strategic priorities. Service Transformation & Pathway Design Lead service redesign around frailty to bring together primary, community, social care and secondary care teams to provide cohesive care at place. Map existing services and resources across the neighbourhood, identifying gaps, duplication, and opportunities for integration. Operational Oversight & Performance Lead As the INT matures, assess and evaluate needs to ensure a transition to operational leadership, overseeing day-to-day service delivery and workforce management. Establish and monitor performance dashboards, using key metrics to measure impact, track progress, and inform continuous improvement. About the RoleThis role is based in the Three Harbours and Bosvena Integrated Neighbourhood team area, working with primary care services covering the patient footprint for Bosvena Health, Fowey River Practice, Lostwithiel Medical Practice and Middleway Surgery—approximately 43,500 people in the catchment. The role is a senior leadership position within the INT steering group, collaborating with commissioners from the Integrated Care Board, General Practice Clinical Leadership, Adult Social Care, Community Care (Cornwall Foundation Trust), Voluntary Sector organisations, and other health and wellbeing services in the community. The role will work peripatetically within the neighbourhood linking teams and forging strong links with secondary care colleagues to provide joined-up healthcare at place.
