Help young people move confidently into adulthood. Were looking for a passionate and skilled Social Worker to join our Transitions Service, supporting young people aged 1725 as they move from childrens services into adult social care. This is a rewarding opportunity to make a real difference at a crucial stage in their lives. About the role As a Social Worker in our Transitions team, you will: Carry out strengths-based, person-centred Care Act assessments. Be the named worker for young people transferring into adult services. Build strong, trusting relationships with young people and their families while working alongside childrens social care. Develop support plans that promote independence and ensure eligible needs are met. Review plans regularly, ensuring least-restrictive, outcome-focused practice. Work collaboratively with professionals across directorates, partners and external agencies. Identify and manage risks, safeguarding young people and vulnerable adults. Keep accurate, timely records on our social care systems. Provide guidance and informal supervision to colleagues and contribute to the development of the wider team. This role plays a key part in supporting young people to achieve their potential and enter adulthood with the right care in place. About you You will be a Qualified Social Worker with SWE registration and experience supporting young people or adults with diverse and complex needs. Youll bring: Strong understanding of relevant social care legislation and safeguarding practice. Experience carrying out assessments and managing complex caseloads. Excellent communication and interpersonal skills. Ability to work collaboratively and build effective partnerships. Strong organisational skills, accurate record keeping and confidence using IT systems. A reflective, proactive approach with commitment to your own professional development. Experience working with people with learning disabilities, autism, mental health needs or SEND is desirable but not essential. Why join us?
