Job Title:Social Worker - Community Wellbeing Team. Location:Stoke-on-Trent Pay Rate:£38.14 per hour About the Role: Liquid Personnel is seeking an Adult Social Worker to join our client's Community Wellbeing team based in Stoke-on-Trent. As an Adult Social Worker within the Community Wellbeing Team, you will support adults aged 18+ with care and support needs arisingfrom physical disabilities, mental health conditions, learning disabilities, or age-related challenges. The role focuses on early intervention, person-centred planning, and strength-based approaches to promote independence and wellbeing in the community. What will your responsibilities be? In this role, you will Carry out Care Act 2014 assessments to identify eligible needs and support planning. Apply strengths-based and person-centred approaches to identify individual goals and outcomes. Coordinate and review support packages, ensuring they are proportionate, cost-effective, and meet statutory requirements. Complete safeguarding enquiries under Section 42 where concerns arise. Undertake Mental Capacity Act assessments and support Best Interest decision-making where appropriate.
