Primary Care Sheffield (PCS) is a GP‑led organisation. Our shareholders are all the GP practices in Sheffield covering over half a million patients. PCS manages nine practices in Sheffield across four localities and four PCNs.ResponsibilitiesMake yourself available to undertake duties in accordance with the practice timetable (surgery consultations, telephone consultations, queries, visiting patients at home, checking and signing repeat prescriptions, paperwork and correspondence).Assess the health care needs of patients with undifferentiated and undiagnosed problems.Screen patients for disease risk factors and early signs of illness.Develop care plans for health in consultation with patients and in line with current practice disease‑management protocols.Record clear and contemporaneous consultation notes.Prescribe in accordance with the practice prescribing formulary (or generically) whenever clinically appropriate.Attend all normal duties and responsibilities associated with a GP.Participate in clinical audit and SEA.Offer support to other clinicians.Participate in appraisal.This is not an exhaustive list of duties and responsibilities. In discussion with the manager the post‑holder may be required to undertake other duties which fall within the definition of the post.
