5 days ago Be among the first 25 applicants The post holder will work within the Crisis Resolution and Home Treatment Team for Older People (CRHTT-OP North) in Peterborough which includes the provision of Dementia Intensive Support (DIST) for people who live with challenging behavioural and psychological symptoms of dementia.The team cares for people who live in the Peterborough, Huntingdon and Fenland areas and the post holder will be required to work in a variety of locations such as nursing / residential homes and the patient’s own home.The post holder will be an experienced practitioner required to take an active role in implementing behavioural management techniques for people who live with dementia and be able to demonstrate those techniques to both paid and unpaid carers. Experience of supporting people and their carers living with dementia is essential.The post holder will manage delegated tasks for several service users, whilst under the supervision of a Band 5 / 6 Mental health nurse/practitioner or above.You will be supporting the team’s CPNs by attending Initial Assessments for patients who are experiencing Behavioural and psychological symptoms of dementia. Patients will be seen in care home environments as well as in their own homes.You will work closely with carers and family and help identify what will help the person and their carers to live well with dementia. You will work closely with care coordinators as well as working independently offering regular intensive care support, identifying triggers to expressed behaviours, role modelling, and psycho education.
