This hybrid position offers regional travel, autonomy, and genuine career progression.Key ResponsibilitiesManage a caseload of community complex-care clients across the Coventry region, ensuring safe and effective delivery. Carry out clinical assessments, risk reviews, and care-plan updates in line with best practice and CQC standards. Provide clinical supervision, mentorship, and competency sign-offs for carers and support staff. Conduct clinical audits, medication checks, and quality reviews to maintain compliance and governance. Liaise with families, ICBs, case managers, and MDT professionals to coordinate holistic care. Deliver training and clinical support to teams during both routine and emergency situations.
