SUPPORT PLANNER ELIZABETH HOUSE, NOTTINGHAM, PERMANENT, 37HRS, GRADE 3 AS, 26,838 , FTE We are looking for a Support Planner to join us! Are you an empathetic, compassionate, and self-motivated? Are you willing to work hard alongside colleagues who want to make a difference to the lives of people in society who need it? Elizabeth House is a well-established service based in Woodthorpe Nottingham, providing accommodation and support for homeless people often with a range of complex support needs. It is a 24-bed hostel with 59 dispersed flats in the community. We are looking for a reliable, enthusiastic and self-motivated Support Planner who can demonstrate an understanding of the issues facing those affected by homelessness. You will join a close knit and loyal team who all bring their own strengths to the service. You dont need to have prior Support Worker experience but must be willing, empathetic, a creative communicator and have good computer skills. Hours are 8am to 4pm with 1 in 5 weeks being on late shifts and having to work 1 weekend in 6. As some of our dispersed housing is not local to the main hostel, it would be great if you drive and have access to a vehicle (cost of travel is paid). However, we also pay the cost of public transport used to carry out the role. Duties & Responsibilities To provide support to Service Users in a safe and well maintained environment. To record support and risk management plans To review and assess resident's individual support needs. To ensure that tasks identified in support plans are agreed with the resident and shared with the team to ensure tasks are carried out. You may have caseloads with Tenants in properties across the city (mileage system for car drivers and costs covered when using public transport). Sign post Tenants onto appropriate services for substance misuse management & health. Assist service users with housing applications and move on options. Assist service users with basic budgeting and life skills in order to support them to move on to live independently. To liaise appropriately with Key partners, stakeholders and other service providers to ensure the best package of support is offered to all Service Users. Why Join Us?
