Locum Adult Social Worker – Continuing Health Care ProjectNewcastle City CouncilOverview: Newcastle City Council is seeking an experienced and motivated Locum Adult Social Worker to support a dedicated 6-month project focused on providing additional capacity to review Continuing Health Care cases. This is a critical role contributing to high-qualityservice delivery and timely decision-making for adults with complex health and care needs. Key Responsibilities:Plan and participate in Decision Support Tool (DST) / Multi-Disciplinary Team (MDT) meetings. Coordinate and confirm attendance of relevant parties at DST meetings. Prepare for DST meetings by gathering relevant case information from Eclipse (case management system), care providers and the individual/family. Attend DST meetings and document outcomes accordingly:If Primary Health Need (PHN) is agreed, record outcome on Eclipse – No Further Action. If PHN is not agreed and disagreement exists:Formally communicate disagreements to the Integrated Care Board (ICB). Attend reconvened DSTs if necessary.
