Key Responsibilities: Conductmental health assessmentsand develop care plans in line with individual needs. Work with service users experiencing a range of mental health conditions, supporting theirrecovery and well-being . Collaborate withNHS teams, private healthcare providers, social services, and voluntary organizations . Providesafeguarding supportand risk management for vulnerable individuals. Support hospital discharge planning, crisis intervention, and community reintegration. Maintainaccurate case recordsand ensure compliance with relevant legislation. Requirements: Degree in Social Work(or equivalent qualification). Registered with Social Work England (or relevant body) . Experience inat least oneof the following areas:NHS, private hospitals, councils, or community-based mental health services . Knowledge ofrelevant legislation(Mental Health Act, Care Act, Mental Capacity Act). Ability towork independently and as part of a multi-disciplinary team .
