Our primary pathway is IV therapy, although you may also support with welfare, monitoring patient conditions, blood tests, and packages of care where required.You will monitor and record patient progress using digital systems, escalate concerns when necessary and work closely with GPs, consultants and multidisciplinary teams to ensure patients receive the right care at the right time.Most importantly, you will help empower patients to recover safely and independently at home, reducing the need for hospital admission and supporting earlier discharge.About YouYou will be a Registered Nurse with a valid NMC registration and at least three years post-registration experience.You will be confident working autonomously within the community, with strong clinical judgement and excellent communication skills.
