The Role: The post holder will be working within Doncaster with support services for vulnerable adults, particularly those individuals of an offending background and complex needs. Responsibilities Include: Housing management Engaging with referral agencies to generate referrals for the service Undertaking appropriate assessments for potential service users and to review and update these as required Completing all documentation including tenure agreements, property inventory, disclosure forms and an application for housing benefit Maximizing rent collection, ensuring successful housing and other welfare benefit claims are submitted and maintained Maintaining contact with service users in accordance with the requirements of the service Designing and delivering an effective person-centred support plan Completing and regularly reviewing safety plans Meeting service users in the community and at home and, where necessary, accompanying them to appointments with relevant agencies Developing professional relationships with a range of external agencies Supporting service users to integrate into the community Developing a move-on plan for service users Keeping clear and comprehensive records and case notes Qualifications / Requirements: A valid UK driving license and access to own vehicle. Business insurance will be required. Enhanced DBS Check In a Typical Week: You will be allocated a caseload of clients who you will support for the agreed contractual hours. We have offices in each area where you can collaborate with other colleagues and managers. You can discuss your client needs and issues as support for yourself and others. Working hours are generally 9am to 5pm with a 20-minute unpaid lunch break. We Offer: A starting salary of 25,715.00 after a full 1 years service this will rise to 27,025.00 and further years service, this will rise to 27,995.00 You will get an initial 28 days Annual Leave with an additional day a year up to 33 days leave, PLUS 8 Bank Holidays.
