You will share the CARE values of the trust and work within a close-knit group of people who bring the feeling of community to the team through their hard work and dedication to the people of the area. The role will be to provide advanced clinical nursing support for patients with complex long-term conditions, assessing and implementing treatments of care. Working as an autonomous practitioner, the role will support the management of our most complex patients, and provide clinical leadership. Therefore, candidates with extended learning and experience of long-term conditions, end of life or wound care will be preferred and the successful candidate will need to have community specialist practitioner district nurse qualification.
