The post holder will play a key role in enhancing the resident experience in care homes by facilitating, enabling and supporting the nursing workforce in their delivery of high quality, competent, evidence based and person-centred care. They will support the practice development facilitator to ensure the aims of the team are met.The post holder will work closely with the team to take account of nurse’s knowledge and skills requirements, competence, performance and development needs so to enable delivery of safe, effective care within a supportive framework. The post holder is expected to demonstrate to nursing registrants excellent clinical nursing practice using simulation methods, e.g. male, female and suprapubic catheterisations, syringe pump management, venepuncture and PEG Tube care and facilitate bespoke education programmes to care homes as identified education needs arise.The post holder will focus on facilitating an optimal resident journey and experience and will ensure the delivery of evidence-based practice and person-centred care taking account of staff knowledge and skills requirements, education, competence and performance against strategic and regional guidance in the management of resident’s conditions.Close links and partnership working across the organisation in acute and community with a range of professionals is essential so that an integrated approach to person-centred care across nursing care settings is provided and sustained.Using and analysing various sources of data and a range of information systems, the post-holder will co-ordinate and manage work priorities utilising a case finder approach to care delivery so as to optimise the resident experience. ResponsibilitiesTo support the Practice Development Facilitator in developing a partnership approach across the organisation and with care home nursing staff to establish an environment which facilitates high quality person-centred nursing care and enhances reside. To follow care home residents’ pathway so as to establish any experiences that may require improvement using practice development methodologies with all staff involved in that resident journey. To assist Practice Development Facilitators and care home staff to ensure effective management of residents with specific conditions and use of anticipated care planning/clinical pathways to ensure a safe and effective resident journey. Have at least 1 years’ experience delivering professional and /or clinical training e.g. professional issues training, application of technical/ clinical skills training.
