Support early intervention and complex discharges from acute site for highly vulnerable patients, specialising in providing occupational therapy assessment and intervention for managing frail, elderly patients with fractures (especially hip fractures), which improves outcomes and reduces costs. Role 3Band 7 Stroke OT PPH. 0800-1600, 37.5 hours per week. The team supports early intervention and complex discharges from the acute site for highly vulnerable patients. This includes specialist occupational therapy assessment and intervention for people who have had a stroke, including assessing reduced function in cognition and motor skills impacting all daily life tasks with a higher number who have still been working when stroke event has happened, reduced function impacts all aspects of life.
