We are excited to announce multiple opportunities for Social Worker (Adults, Care and Support) positions within a newly created team operating on a countywide basis. This is an excellent chance to be part of a dynamic team dedicated to responding swiftly to referrals and providing vital assessments and support. As a Social Worker (Adults, Care and Support), you will play a crucial role in delivering high-quality care assessments across Cornwall. Your responsibilities will include carrying out Care Act assessments, care and support planning, and Mental Capacity assessments, ensuring individuals receive the support they need.
