About the Role We are seeking a compassionate, experienced Registered Nurse (Adult or Learning Disability) to join our team, enabling us to deliver high-quality, person-centred care to clients with complex health needs in the community.This role involves training staff and resolving conflicts/emergencies within supporting individuals with long-term physical disabilities and learning disabilities, including cerebral palsy, epilepsy, multiple sclerosis (MS), stroke recovery, continence issues, and accident-related physical impairments . You will work closely with support staff, families, and external health professionals to ensure each client receives safe, effective, and responsive care. Key Responsibilities Deliver and advising on nursing support and care in line with individualised care plans, risk assessments, and best practice standards. Conduct nursing assessments and clinical observations. Provide expert guidance on: Continence management Cerebral palsy and spasticity care Epilepsy and seizure support (including emergency plans) Post-stroke rehabilitation Mobility-limiting diseases (e.g., MS, spinal injuries) Learning disabilities and complex behaviour Service user crisis Management Liaise with GPs, therapists, and multidisciplinary teams (MDTs). Monitor for changes in clinical status and escalate appropriately. Support and upskill care staff through training and supervision. Ensure accurate, timely documentation in line with CQC requirements. Promote dignity, choice, and independence for all clients. What Were Looking For Registered Nurse (Adult or RNLD) with active NMC PIN required. Minimum 3 years post-registration experience in nursing. Proven experience working with complex needs, physical disability, and/or learning disabilities . Strong clinical assessment and risk management skills. Mentoring or staff development experience. PEG/gastrostomy care Excellent verbal and written communication. Confident in working independently and as part of a multidisciplinary team. Passionate about delivering person-centred care in a community setting. Desirable (But Not Essential) Experience as a community nurse, case manager, or complex needs nurse .
